Your GP health record contains information about the care you receive at Manor Medical Practice, including your medical history, medicines, test results and other information relevant to your care.
You have a right to access information held about you, and there are several ways you can view or request your health information.
View your health record online
The NHS App is the easiest way for many patients to view information held in their GP health record.
Depending on the access available to you, you may be able to view:
- medicines and allergies
- test results
- vaccinations
- consultations and entries in your record
- letters and documents
Some older information may not automatically be available through the NHS App. If you need information that you cannot see online, please contact the practice.
Request a copy of your health record
You have the right to request access to personal information that we hold about you. This is known as a Subject Access Request (SAR).
You can request all or part of your health record.
To make a request:
Please tell us what information you would like to access. We may contact you if we need further information to understand your request or to confirm your identity. There is normally no charge for a Subject Access Request.
We will respond without undue delay and normally within one month. In some circumstances, such as where a request is complex, this period may be extended. We will let you know if this applies.
Accessing someone else's health record
We have a duty to protect the confidentiality of every patient.
If you would like access to another person’s health information, we will normally need their consent or appropriate legal authority before information can be provided.
There may be different arrangements for children, people who are unable to provide consent, or someone acting with appropriate legal authority.
Please contact our reception team if you would like to discuss accessing information on behalf of someone else.
Proxy access
Proxy access allows another person to use GP online services on behalf of a patient.
This can be helpful, for example, for a parent, carer or family member who helps someone manage their healthcare.
Depending on the access agreed, a proxy may be able to help with things such as ordering repeat prescriptions, managing appointments or viewing parts of the patient’s health record.
The level of access provided will depend on the individual circumstances and appropriate consent or authority.

Something in my health record is incorrect
If you believe information recorded about you is inaccurate, please let us know.
We can investigate information that may have been recorded incorrectly and make corrections where appropriate.
Some information, particularly clinical information, cannot simply be deleted or changed because you disagree with it. Where appropriate, your concerns or additional information can be recorded alongside the original entry.
Please contact the practice if you believe information in your record needs to be reviewed.
How we use and share your information
We have a duty to keep your personal and health information confidential and secure.
We use information about you to provide your healthcare and may share information where appropriate for your care or where there is another lawful reason to do so.

National Data Opt-Out
The National Data Opt-Out allows you to choose whether your confidential patient information can be used for certain purposes beyond your individual care, such as research and planning.
Choosing to opt out does not affect your individual care or treatment, and your information can still be used where there is a legal requirement or another permitted reason.
You can check or change your choice at any time. Your choice applies across the NHS rather than just at Manor Medical Practice.
You can manage your choice:
- Online through the NHS National Data Opt-Out service
- Using the NHS App – go to Your health → Choose if data from your health records is shared for research and planning
- If you cannot use the digital services, the NHS also provides alternative ways to make or change your choice.
