Register With Our Practice

We welcome new patients who would like to register with Manor Medical Practice.

The quickest and easiest way to register is online using the NHS Register with a GP Surgery service.

The registration form will ask for information about you, your previous GP practice and your health. Once we receive your application, our team will process it and contact you if we need any further information.

Our Practice Area

Our usual practice area is shown on the map below. You can enter your postcode to check whether your home address is within our practice boundary.

Live Outside Our Practice Area?

You can still apply to register with Manor Medical Practice if you live outside our usual practice area. Applications from outside our practice area will be considered to make sure that registering with us is clinically appropriate and practical for you.

If you register with us as an out-of-area patient, there are some important differences you should be aware of. In particular, Manor Medical Practice would not normally be responsible for providing a home visit when you are at home and unable to attend the practice. Registering outside your local area may also affect how you access urgent or out-of-hours primary care and some community services, such as physiotherapy and maternity services.

For this reason, if you live some distance from Manor Medical Practice, you may wish to consider registering with a GP practice closer to your home so that you can more easily access the full range of local primary and community healthcare services.

If you choose to apply to register with Manor Medical Practice from outside our practice area, we will consider your application and contact you if we need to discuss anything with you.

If You Cannot Register Online

If you are unable or do not wish to use the online registration service, please speak to our reception team. We can provide a paper registration form and help you with the registration process if needed.

You do not need to provide proof of address, photographic identification, proof of immigration status or an NHS number to register with a GP practice. If you know your NHS number and details of your previous GP practice, providing these can help us identify your existing health record.

New Patient Health Checks

Once you have registered, you may be invited for a new patient health check. This provides an opportunity for us to review your health and identify any ongoing healthcare needs.

The appointment may include checks such as your height, weight and blood pressure, as well as questions about your health, medication and lifestyle.

If English is not your first language, we can arrange an interpreter where appropriate. Please also let us know if you have any communication needs, accessibility requirements or require a reasonable adjustment to help you register or access our services.

Your Named GP

All registered patients are allocated a named GP who has overall responsibility for coordinating their care.

Having a named GP does not restrict who you can see at the practice. You can continue to receive care from other GPs and members of our healthcare team.
