Travel Health
If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required. We will ask you to complete a travel vaccination form and book a telephone triage appointment with our Practice Nurse to discuss requirements. A further face to face appointment will be required if the Practice Nurse needs to administer any vaccinations

If you are travelling abroad please complete and submit the travel form in plenty of time (we recommend 6-8 weeks) before your journey.
