Register with our practice

We are now offering an online registration service through NHS Digital. This means you can complete a form online and submit it to the surgery without the need for filling out a paper form.

If you are unable to do this you can click here to download and print a paper version or collect a paper form from the surgery.

If you are registering a child online you will be sent an additional form to complete for further information.

Named GP

All registered patients will be assigned to one of the GP Partners. You have the right to express a preference of which GP partner you are assigned to, and you can request to see any practitioner, regardless of who you are assigned to.

Temporary Patient Registrations

If you are away from home and have a new or acute problem you will need to attend the Urgent Treatment Centre in Lymington.

If you need a repeat prescription you will need to contact your own surgery and they will be able to send it electronically to a local pharmacy here. You will need to remember to change it again with your GP when you return home.

If you feel that none of the above are appropriate then please contact the surgery.

Detailed access to your record

Once you are signed up to patient access, you have the option to request detailed access to your record. You will be able to see the following information online:

  • Test results
  • Problems
  • Documents

To request this, call us on 02380 282689 and ask for a ‘detailed access request form’, you will then need to drop the form into the surgery with photo identification.

Please allow 14 days for your request to be processed.

Proxy access

Proxy access allows a parent, relative or carer to manage online services on behalf of another patient.

There are 3 age groups which need different forms completing:

  • Children 0-10 – Parent/Guardian/Carer can complete the form and return to the surgery
  • Children 11-15 – The child can opt for having their own access (but must have their own mobile number and email address), the child can opt of their parent/guardian/carer to have access or the child can opt of both to have access.
  • Adults 16+

The form needs completing with all parties having signed (except the first group).
