Register with our practice

The quickest and easiest way to resister with Mansfield Medical Centre is to complete our online registration form.

If you are unable to complete the online registration form you can register with the surgery by completing a registration form (PRF1) which can be downloaded here and bringing this into the surgery, or ask for a paper copy from reception.

If you are registering using the paper form please bring the following information with you:

  • Your NHS number (If you aren’t sure you either can ask your previous GP surgery, check on a previous medical letter or use Find your NHS number)
  • Proof of identification (eg Passport, Driving Licence, Home Office ID card or European ID card)
  • Proof of address (eg Utility or Bank statement dated within the last 3 months)
  • If you are registering a newborn baby please also bring the Red Book

At Mansfield Medical Centre we do accept out of area registrations, however there are some limitations on the services you will be eligible for (please enter your details in the post code checker below [top right corner of the map below], to see if you live within the catchment area of the surgery).

If you move outside the catchment area you can choose to remain a patient at Mansfield Medical Centre, however you will be re-registered as an Out of Area patient.

Named GP

Patients have been allocated an accountable GP.  Patients can obtain the name of their accountable GP by contacting the practice or by asking at your next appointment. This does not affect your choice of GP when attending appointments.

Access my medical records

The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. If you wish to request access to the records held by the practice, please contact our reception staff on 024 7645 7551.
