Who do I see?
Cancelling your appointment
It is important that you inform the reception staff if you are unable to attend your appointment, this will allow that appointment to be offered to another patient. If you fail to notify the Practice that you are unable to attend, you will be sent an sms message informing you that you have defaulted from your appointment. Persistent defaulters maybe removed from the list.
You can cancel your appointment by:
- Phoning the Surgery
- Submitting a Klinik
- Emailing the Surgery
If it is short notice, phone calls are the Surgery’s preferred method of cancellation. We receive hundreds of emails each day and a request to cancel an appointment may not be seen until after the appointment time, meaning we are unable to allocate it to another patient.

For appointment requests, please complete a Klinik form below, where you can choose from the following options:
- New health problem
- Existing health problem
- Mental health
- Routine Nurse/HCA appointments
- Cancelling an appointment
If you are unable to complete a Klinik form, please call the Surgery and one of our helpful Reception Team will complete the form over the phone with you.

Start hereExtended Hours Scheme (St Levan Surgery ONLY)
We are offering routine appointments under the Extended Hours Scheme every other Saturday from 09:00 – 12:00 and some evenings until 19:00.
All of these routine appointments are pre-booked only. Evening appointments will be telephone only with a GP or Clinical Practitioner.
Please contact the surgery to book one of these appointments for things such as: Diabetes Reviews, Asthma Checks, Coil Fits, Minor Surgery, Cervical Screening.
These extended hours pre-booked appointments are not for acute care. When a patient is unwell and unable to wait for normal opening hours – Please call 111 for this service (Or 999 if required in an emergency).
Home Visits
If possible please try to contact us before 10am if you require a home visit – please follow the usual method of contact, either via Klinik, over the phone, or in-person at the Surgery.
A clinician may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a nurse, or indeed arrange a hospital attendance.
House visits are only available for patients who are housebound because of illness or disability and are at the discretion of the GP.
Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Surgery.
