Young People’s Health
The Young Person’s Social Prescription Service (ASPS) was created in 2019 to support young people aged 11-25. Its central aim is to provide support to young people who require access to community-based services that can help them resolve the mental health conditions and general wellbeing issues.
The aims of this service are to:
- To signpost young people and their families to appropriate community-based services after they have been assessed by a Stort Valley and Villages PCN GP.
- Support general wellbeing amongst young people and their families in the local community
- Engage in research that highlights how effective community interventions can be within PCN trusts and submit our findings to NHS and government leaders
- To offer preventative interventions such as the Family Wellbeing Health Coaching Service and to work alongside other services with a view to creating activities and groups for those who have been referred.
This service offers a range of interventions such as online wellbeing education groups, online art groups and consistent support and guidance through the whole process.
Ben Nesham, Young Person’s Social Prescriber and Link Worker for Stort Valley PCN, directs this service.
If you are registered with one of the Stort Valley and Villages Primary Care Network surgeries, you are eligible for this support. However, you will be required to first visit your GP and ask for a referral to this service.

The team can provide support for Herts residents who are experiencing a mental health crisis, need some mental health support or just want to talk.
