It includes important information about your health:<\/p>\n<ul>\n<li class=\"font_8 wixui-rich-text__text\"><span class=\"wixGuard wixui-rich-text__text\">\u200b<\/span>Medicines you are taking<\/li>\n<li class=\"font_8 wixui-rich-text__text\">Allergies you suffer from<\/li>\n<li class=\"font_8 wixui-rich-text__text\">Any bad reactions to medicines<\/li>\n<\/ul>\n<p class=\"font_8 wixui-rich-text__text\">You may need to be treated by health and care professionals that do not know your medical history. Essential details about your healthcare can be difficult to remember, particularly when you are unwell or have complex care needs. Having an SCR can help you if you need healthcare as the staff involved in your care make better and safer decisions about how best to treat you.<\/p>\n<p class=\"font_8 wixui-rich-text__text\">If you wish to opt out at any time, please ask reception for a form.<\/p>\n<h3>Additional Information<\/h3>\n<p class=\"font_8 wixui-rich-text__text\">You can choose to have additional information included in your SCR, which can enhance the care you receive. This information includes:<\/p>\n<ul>\n<li class=\"font_8 wixui-rich-text__text\">Your illnesses and health problems<\/li>\n<li class=\"font_8 wixui-rich-text__text\">Operations and vaccinations you have had in the past<\/li>\n<li class=\"font_8 wixui-rich-text__text\">How you would like to be treated \u2013 such as where you would prefer to receive care<\/li>\n<li class=\"font_8 wixui-rich-text__text\">What support you might need<\/li>\n<li class=\"font_8 wixui-rich-text__text\">Who should be contacted for more information about you<\/li>\n<\/ul>\n<p class=\"font_8 wixui-rich-text__text\">If you would like to have additional information included in your SCR, please ask at reception for a form.
