{"id":213,"date":"2023-10-06T10:17:50","date_gmt":"2023-10-06T09:17:50","guid":{"rendered":"https:\/\/nhs-demo.surgeryweb.org.uk\/?page_id=213"},"modified":"2026-10-06T10:19:52","modified_gmt":"2026-10-06T09:19:52","slug":"sick-fit-notes","status":"publish","type":"page","link":"https:\/\/surgerywebtemp.org.uk\/l83646\/sick-fit-notes\/","title":{"rendered":"Sick (fit) notes"},"content":{"rendered":"<h3>If you&#8217;ve been sick for 7 days or less<\/h3>\n<p>Use form SC2 if you are an employee, you\u2019ve been sick for 1 or more days in a row and need to claim <abbr data-toggle=\"tooltip\" data-placement=\"bottom\" data-bs-original-title=\"Statutory Sick Pay\">SSP<\/abbr>\u00a0from your employer.<\/p>\n<p>When you have filled in the form give it to your employer, it will help them decide if you can get\u00a0<abbr data-toggle=\"tooltip\" data-placement=\"bottom\" data-bs-original-title=\"Statutory Sick Pay\">SSP<\/abbr>. Keep a copy for your own information.<\/p>\n<p>Please follow this link to complete the self certification form:<\/p>\n<p><a href=\"https:\/\/www.gov.uk\/government\/publications\/statutory-sick-pay-employees-statement-of-sickness-sc2\">Employee\u2019s statement of sickness to claim Statutory Sick Pay &#8211; GOV.UK (www.gov.uk)<\/a><\/p>\n<h3>If you&#8217;ve been sick for <span style=\"text-decoration: underline;\">more<\/span> than 7 days<\/h3>\n<p>If you have been ill for more than 7 days you will need a doctor\u2019s certificate. These certificates are called \u2018Fitness to Work\u2019 Certificates.<\/p>\n<p>Please submit a Klinik via the below link to request this, or alternatively, please call the Surgery.<\/p>\n<p><a href=\"https:\/\/access.klinik.co.uk\/contact\/st-levan-surgery\">Klinik &#8211; request a sick note<\/a><\/p>\n<p>PLEASE NOTE SICK\/ FIT NOTE CERTIFICATES CAN TAKE UP TO <strong>72 WORKING HOURS\/ 3 WORKING DAYS<\/strong> TO PROVIDE<\/p>\n<h3>Requesting an additional sick\/fit note<\/h3>\n<p>If you have already had a Sick Note (Fit Note) for this illness your Doctor may not need to see you to issue an additional Sick Note.
