Register with our practice

To register as a patient at Bloxham and Hook Norton Surgeries your home address must fall within our practice boundary, please contact the Surgery to see if you are in our area.

To ensure that we are providing the appropriate standard of care to our patients, including home visits, we are only able to accept new patients from within our boundary area.

To register you need to complete the New patient Registration form – see below link.

When completing the forms please make sure you are completing as much of the information as possible, including your NHS number. If you dont know your NHS number you can contact your existing GP for this or you can find it here:  Find your NHS number – NHS

The registration process takes 5 working days unless there is information missing on the form.

Overseas Visitors are entitled to free NHS GP services once they register with the Surgery on a permanent basis, this is different for visitors.  If you are a visitor please find information here:

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
