Patients must consent to sharing of the data held by a health provider out to other health providers and must also consent to which of the other providers can access their data.<\/p>\n<p>e.g. it may be necessary to share data held in GP practices with district nurses but the local podiatry department would not need to see it to undertake their work. In this case, patients would allow the surgery to share their data, they would allow the district nurses to access it but they would not allow access by the podiatry department. In this way access to patient data is under patients&#8217; control and can be shared on a &#8216;need to know&#8217; basis.<\/p>\n<h3>Access your medical records<\/h3>\n<p>To access the EMIS\u00a0on-line portal, click <a href=\"https:\/\/account.patientaccess.com\/Account\/Login?\" target=\"_blank\" rel=\"noopener noreferrer\" data-link-id=\"115598\" data-use-default-display=\"false\" data-use-default-title=\"false\" data-toggle=\"tooltip\" data-placement=\"bottom\" data-bs-original-title=\"here\">here<\/a>.<\/p>\n<p>Please visit the support pages for guidance on registering for and using your online account:\u00a0<a href=\"https:\/\/support.patientaccess.com\/\">Patient Access Support Portal<\/a><\/p>\n<p>You can access key parts of your medical record on-line, including test results and medications through the portal. You can also book apppointments through it.<\/p>\n<p>You&#8217;ll need to register for the portal in the surgery so that we can verify your identity and provide you with a secure password. Please ask at reception.<\/p>\n<h3 style=\"font-weight: 600;\">Planning on moving abroad?<\/h3>\n<p style=\"font-weight: 400;\">If you\u2019re moving abroad on a permanent basis, you\u2019ll no longer automatically be entitled to medical treatment under normal NHS rules. This is because the NHS is a <strong style=\"font-weight: 600;\">residence-based healthcare system.<\/strong><br \/>\nYou\u2019ll have to notify your GP practice so you and your family can be removed from the NHS register.<\/p>\n<h4 style=\"font-weight: 600;\">Your Medical Record<\/h4>\n<p style=\"font-weight: 400;\">If you are planning on living overseas it is important you have a copy of your medical record to take with you.<\/p>\n<p style=\"font-weight: 400;\">We can provide patients with a paper copy of their medical record, and in some cases a digital copy. <strong style=\"font-weight: 600;\">Requests should be made as soon as possible<\/strong> and preferably <strong style=\"font-weight: 600;\">before they leave the country<\/strong> to ensure we have time to request paper record from storage, make copies of these, as well as produce a copy of the electronic record. Patients cannot be given their original paper record.<\/p>\n<p style=\"font-weight: 400;\">Your medical record <strong style=\"font-weight: 600;\">cannot be released<\/strong> to you until we have <strong style=\"font-weight: 600;\">verified your identity<\/strong> \u2013 you will need to present to the practice with photographic ID and proof of address. Typically this would be your Passport and Driving Licence.<\/p>\n<h4 style=\"font-weight: 600;\">What happens to patient records after they leave?<\/h4>\n<p style=\"font-weight: 400;\">Paper records are stored by the PCSE (Primary Care Support England) for 10 years after the patient has left the country.<\/p>\n<p style=\"font-weight: 400;\">The digital record is currently kept indefinitley, however access to deducted notes is restricted and auditable.<\/p>\n<h3 style=\"font-weight: 600;\">Out of the country for more than 3 months of the year?<\/h3>\n<p style=\"font-weight: 400;\">We are required to deduct any patient from our list if they are living outside the of the UK for <strong style=\"font-weight: 600;\">more than 3 months of the year<\/strong>.
