Your Information – what you need to know<\/h2>\n

The\u00a0privacy notice explains why\u00a0Selsey Medical Practice\u00a0collect information about you, how that information will be used, how we keep it safe and confidential and what your rights are in relation to this.<\/p>\n

Why we collect information about you.<\/h2>\n

Health care professionals who provide you with care are required by law to maintain records about your health and any treatment or care you have received.\u00a0 These records help to provide you with the best possible healthcare and help us to protect your safety.<\/p>\n

We collect and hold data for the purpose of providing healthcare services to our patients and running our organisation which includes monitoring the quality of care that we provide. In carrying out this role we will collect information about you which helps us respond to your queries or secure specialist services. We will keep your information in written form and\/or in digital form.<\/p>\n

Please be aware that all incoming and outgoing telephone calls to and from the practice are recorded.<\/p>\n

Call recordings are used for legitimate business purposes, including staff training, quality assurance, service improvement, patient and staff safety, and the investigation of complaints or incidents.<\/p>\n

Recordings are processed and stored securely in accordance with UK data protection legislation and the NHS Records Management Code of Practice. Access is restricted to authorised personnel only.<\/p>\n

\n\t\t\t\n\t\t\t\tChaperones<\/strong><\/span>\n\t\t\t<\/summary>\n\t\t\t
<\/p>\n

We will always respect your privacy, dignity and your religious and cultural beliefs particularly when intimate examinations are advisable – these will only be carried out with your express agreement and you will be offered a chaperone to attend the examination if you so wish.<\/p>\n

You may also request a chaperone when making the appointment or on arrival at the surgery (please let the receptionist know) or at any time during the consultation.<\/p>\n

\n\t\t\t\n\t\t\t\tViolence Policy (Zero Tolerance)<\/strong><\/span>\n\t\t\t<\/summary>\n\t\t\t
<\/p>\n

The NHS operate a zero tolerance policy with regard to violence and abuse and the practice has the right to remove violent patients from the list with immediate effect in order to safeguard practice staff, patients and other persons.<\/p>\n

Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person\u2019s safety.<\/p>\n

In this situation we will notify the patient in writing of their removal from the list and record in the patient\u2019s medical records the fact of the removal and the circumstances leading to it.<\/p>\n

  1. Where\u00a0we have a contract with\u00a0an organisation\u00a0we are sharing with or\u00a0we are obliged to share as in HMRC<\/em> – Article 6(1)(c) \u2013 \u2018processing is necessary for compliance with a legal obligation to which the controller is subject\u2026\u2019<\/li>\n

When\u00a0we share patient information with another provider of health care it is in line with<\/em> –\u00a0Article 6 1 (e )\u00a0 \u2018processing is necessary for the performance of a task carried out in the public interest or in the exercise of official authority vested in the controller\u2019<\/li>\n

  4. We will be sharing different types of data with these organisations, and for a different legal basis;
