Via Meridian PCN, we offer health coaches to our patients.<\/p>\n

They can provide support and motivation for changes in habits (smoking, alcohol, weight loss), particularly in situations where motivation is difficult, such as when suffering from mental health conditions.<\/p>\n

They can also support changing behaviours for those who are isolated.<\/p>\n
