This includes letters, test results and appointment notes.<\/p>\n<p>You can access your health record and nominate someone you trust to access it too.<\/p>\n<p>You can use your <a href=\"https:\/\/www.nhs.uk\/nhs-app\/account\/\">NHS account<\/a> (through the NHS website or NHS App) or <a href=\"https:\/\/account.patientaccess.com\/Account\/Login?ReturnUrl=%2Fconnect%2Fauthorize%2Fcallback%3Fclient_id%3Dpkce_patientaccess_web%26redirect_uri%3Dhttps%253A%252F%252Fapp.patientaccess.com%252Fsignin-callback%26response_type%3Dcode%26scope%3Dopenid%2520profile%2520patientaccess_api%26state%3D38fe56e2348f4988ae65a7d281f79e1a%26code_challenge%3D-CtTwpk30fK64qgXEvmaN6TFgDyHMdPudHZoNIuAK2E%26code_challenge_method%3DS256%26response_mode%3Dquery%26titleType%3Ddefault\">Patient Access<\/a> to access your health record.<\/p>\n<p>You\u2019ll need to ask the surgery for online access to your full health record, or you\u2019ll only see your medicines and allergies.<\/p>\n<p>Alternatively, please print out and complete the below form, to bring in to reception:<\/p>\n<ul class=\"wp-block-list\">\n<li><a href=\"https:\/\/surgerywebtemp.org.uk\/c82009\/wp-content\/uploads\/2026\/01\/Access-to-medical-records-Form.doc\">Request access to medical records \u2013 Form<\/a><\/li>\n<\/ul>\n<h2 class=\"wp-block-heading\">Request access to your full health record online<\/h2>\n<p>First, you need to register for online services and prove who you are. To request access:<\/p>\n<ul class=\"wp-block-list\">\n<li>visit the surgery with photo ID so we can confirm your identity<\/li>\n<li>if you are setting up access for the first time, we will give you a username and password<\/li>\n<li>go to <a href=\"https:\/\/account.patientaccess.com\/Account\/Login?ReturnUrl=%2Fconnect%2Fauthorize%2Fcallback%3Fclient_id%3Dpkce_patientaccess_web%26redirect_uri%3Dhttps%253A%252F%252Fapp.patientaccess.com%252Fsignin-callback%26response_type%3Dcode%26scope%3Dopenid%2520profile%2520patientaccess_api%26state%3D38fe56e2348f4988ae65a7d281f79e1a%26code_challenge%3D-CtTwpk30fK64qgXEvmaN6TFgDyHMdPudHZoNIuAK2E%26code_challenge_method%3DS256%26response_mode%3Dquery%26titleType%3Ddefault\">Patient Access<\/a> or the app for access<\/li>\n<\/ul>\n<p>All patients are able to view their prescriptions, lab results and summary of medical records online via the NHS app or patient access. In most cases, documents are also visible.<\/p>\n<p>If any of your details i.e. email address or telephone number that we have on our records are different\/incorrect, the registration will not work, you will need to either contact the surgery and speak to registrations, or alternatively fill in the form below to enable us to update your details.<\/p>\n<p>Please note this service is currently only available to patients 16 years and over.<\/p>\n<p>Please contact the medical centre registrations team on Market Harborough Medical Centre <a class=\"tel-no\" href=\"tel:01858464242\">01858 464242<\/a>or St Luke\u2019s Branch Surgery <a class=\"tel-no\" href=\"tel:01858464242\">01858 464242<\/a> or Husbands Bosworth Medical Centre <a class=\"tel-no\" href=\"tel:01858880522\">01858 880522<\/a> if you are having any problems getting online or do not have a smartphone.<\/p>\n<h2>Accessing someone else&#8217;s information<\/h2>\n<p>As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.<\/p>\n<p>To request proxy access:<\/p>\n<ul class=\"wp-block-list\">\n<li>fill out an <a href=\"https:\/\/surgerywebtemp.org.uk\/c82009\/online-access-for-proxy\/\">online access for proxy<\/a> form<\/li>\n<li>collect a proxy access form from reception during opening times<\/li>\n<\/ul>\n<h2 class=\"wp-block-heading\">Linked profiles in your NHS account<\/h2>\n<p>Once proxy access is set up, you can access the other person\u2019s profile in your NHS account, using the NHS App or website.<\/p>\n<p>The NHS website has information about\u00a0<a href=\"https:\/\/www.nhs.uk\/nhs-app\/nhs-app-help-and-support\/linked-profiles-in-the-nhs-app\/\">using linked profiles to access services for someone else<\/a>.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Your health record includes information like any conditions or allergies you have and any medicine you\u2019re taking.
