You can ask them questions about your health and tell them how you are feeling.<\/p>\n<details class=\"nhsuk-details nhsuk-expander\">\n\t\t\t<summary class=\"nhsuk-details__summary\">\n\t\t\t\t<span class=\"nhsuk-details__summary-text\">New Patient Health Checks<\/span>\n\t\t\t<\/summary>\n\t\t\t<div class=\"nhsuk-details__text\"><\/p>\n<p><strong>New patients (over the age of 5) will be offered a free health check.<\/strong><\/p>\n<p>The health check helps us to assess and review your health care needs and record clinical data such as height, weight and blood pressure. You will be asked about:<\/p>\n<ul>\n<li>Current and past illnesses<\/li>\n<li>Operations<\/li>\n<li>Illnesses that run in the family<\/li>\n<li>Medication and allergies, including repeat medications that may need to be added to your record<\/li>\n<li>Details of screening tests such as cervical smears<\/li>\n<li>Immunisations you\u2019ve had<\/li>\n<\/ul>\n<p>Any health issues\/concerns to that they will be directed to the appropriate nurse\/clinician if they have any health issues they wish to be discussed\/reviews due<\/p>\n<p><\/div>\n\t\t<\/details>\n<p><a href=\"https:\/\/www.nhs.uk\/medicines\/warfarin\/\">Anticoagulation (Warfarin)<\/a> &#8211; Clayton Patients<\/p>\n<p><a href=\"https:\/\/mylivingwell.co.uk\/\">Weight Management<\/a><\/p>\n<p><a href=\"https:\/\/www.nhs.uk\/conditions\/nhs-health-check\/\">NHS Health Checks<\/a><\/p>\n<p><a href=\"https:\/\/www.nhs.uk\/conditions\/vaccinations\/\">Childhood\/Adult Vaccinations<\/a><\/p>\n<p><a href=\"https:\/\/www.nhs.uk\/conditions\/travel-vaccinations\/\">Travel Vaccinations<\/a><\/p>\n<p><a href=\"https:\/\/www.nhs.uk\/common-health-questions\/accidents-first-aid-and-treatments\/how-should-i-care-for-my-stitches\/\">Removal of Stitches<\/a><\/p>\n<details class=\"nhsuk-details nhsuk-expander\">\n\t\t\t<summary class=\"nhsuk-details__summary\">\n\t\t\t\t<span class=\"nhsuk-details__summary-text\">Dressings<\/span>\n\t\t\t<\/summary>\n\t\t\t<div class=\"nhsuk-details__text\"><\/p>\n<p>This is nurse led for adults, providing care to wounds which are less than 6 weeks old, e.g. surgical incisions, skin tears, abrasions and post operative wounds.<\/p>\n<p>Access is by appointment only. Your GP, practice nurse, district nurse or hospital staff will usually refer you.<\/p>\n<p><\/div>\n\t\t<\/details>\n<p><a href=\"https:\/\/assets.publishing.service.gov.uk\/government\/uploads\/system\/uploads\/attachment_data\/file\/1105217\/UKHSA_Surgical_site_infection_leaflet_A4.pdf\">Wound Swabs<\/a><\/p>\n<p><a href=\"https:\/\/www.nhs.uk\/conditions\/blood-tests\/\">Phlebotomy (Blood Tests)<\/a><\/p>\n<p><a href=\"https:\/\/www.nhs.uk\/conditions\/cervical-screening\/\">Cervical Screening<\/a><\/p>\n<details class=\"nhsuk-details nhsuk-expander\">\n\t\t\t<summary class=\"nhsuk-details__summary\">\n\t\t\t\t<span class=\"nhsuk-details__summary-text\">Vaginal Swabs<\/span>\n\t\t\t<\/summary>\n\t\t\t<div class=\"nhsuk-details__text\"><\/p>\n<p><strong>How to Self-Collect a Vaginal Swab Specimens<\/strong><\/p>\n<p><strong>Please read all instructions before collecting specimens.<\/strong><\/p>\n<p><em>Vaginal Swab Specimen Collection Guide:<\/em><\/p>\n<p><strong>1.<\/strong> Wash hands with soap and water. Rinse and dry.<\/p>\n<p><strong>2.<\/strong> Hold the swab in one hand and with the other hand separate the folds of skin around the vaginal opening (labia). Do not touch the swab tip or lay it down. If you touch the tip or lay the swab down, ask for a new swab.<\/p>\n<p><strong>3.<\/strong> Insert the swab about 5 cm (2 inches) into the vaginal opening. Gently turn the swab for about 30 seconds while rubbing the swab against the wall of the vagina.<\/p>\n<p><strong>4.<\/strong> Withdraw the swab without touching the skin. holding the swab in the same hand remove the cap from the tube. Place the swab in the tube.<\/p>\n<p><strong>5.<\/strong> Tightly close the PCR Media tube.<\/p>\n<p><strong>6.<\/strong> After collection, wash hands with soap and water, rinse, and dry.<\/p>\n<p><strong>7.<\/strong> Return the sample to your healthcare provider as instructed.<\/p>\n<p><\/div>\n\t\t<\/details>\n<p><a href=\"https:\/\/www.nhs.uk\/conditions\/electrocardiogram\/\">ECG&#8217;s<\/a><\/p>\n<p><a href=\"https:\/\/www.nhs.uk\/conditions\/stop-smoking-treatments\/\">Smoking Cessation<\/a><\/p>\n<hr \/>\n<p>Hollyns Health and Wellbeing host a number of additional services for our Patients that are provided by Allied Health Professionals. These are either NHS service providers or Health Care based professionals that are not employed directly by us but offer their services to our Patients within our buildings.<\/p>\n<h3>Pharmacists<\/h3>\n<p>Our Trust Pharmacy Team (made up of pharmacists and pharmacy technicians) who can provide a range of services such as telephone and face-to-face medication reviews, prescription requests, medication queries, repeat dispensing set-up and synchronisation of repeat prescriptions.<\/p>\n<h3 class=\"entry_title\">HALE \u2013 Health Action Local Engagement<\/h3>\n<p>HALE (Health Action Local Engagement) is an award winning charity based in Bradford, who work with local people, enabling them to make lifestyle changes to improve their health through a variety of programmes and activities, focusing on Isolation, Self Care, Alcohol Awareness, Sexual Health \u0003and Nutrition.<\/p>\n<p>HALE work with people of all ages and backgrounds, and pride themselves on having a skilled team of community health workers who understand how to make a difference to people\u2019s health and wellbeing.<\/p>\n<p>HALE provides access to a variety of activities, groups and programmes such as befriending and community connectors that all aim to reduce isolation and improve mental health and wellbeing.<\/p>\n<p>For more information please click on the link: <a href=\"https:\/\/haleproject.org.uk\/bradford-healthy-living-services-directory\/\">HALE<\/a><\/p>\n<h3>First Contact Physiotherapy<\/h3>\n<p>This is an assessment and advice service carried out by a qualified physiotherapist.<\/p>\n<p>It is available to patients <u>over 16 years of age<\/u>, bookable in advance. The service is for new onset muscular joint and back problems to be assessed , it does not provide ongoing treatment.
