{"id":561,"date":"2026-09-28T15:18:48","date_gmt":"2026-09-28T14:18:48","guid":{"rendered":"https:\/\/surgerywebtemp.org.uk\/b83012\/?page_id=561"},"modified":"2026-09-28T15:29:48","modified_gmt":"2026-09-28T14:29:48","slug":"your-health-records-information","status":"publish","type":"page","link":"https:\/\/surgerywebtemp.org.uk\/b83012\/your-health-records-information\/","title":{"rendered":"Your Health Records &#038; Information"},"content":{"rendered":"<p>Your GP health record contains information about the care you receive at Manor Medical Practice, including your medical history, medicines, test results and other information relevant to your care.<\/p>\n<p>You have a right to access information held about you, and there are several ways you can view or request your health information.<\/p>\n<h2>View your health record online<\/h2>\n<p>The NHS App is the easiest way for many patients to view information held in their GP health record.<\/p>\n<p>Depending on the access available to you, you may be able to view:<\/p>\n<ul>\n<li>medicines and allergies<\/li>\n<li>test results<\/li>\n<li>vaccinations<\/li>\n<li>consultations and entries in your record<\/li>\n<li>letters and documents<\/li>\n<\/ul>\n<p>Some older information may not automatically be available through the NHS App. If you need information that you cannot see online, please contact the practice.<\/p>\n<h2>Request a copy of your health record<\/h2>\n<p>You have the right to request access to personal information that we hold about you. This is known as a Subject Access Request (SAR).<\/p>\n<p>You can request all or part of your health record.<\/p>\n<p>To make a request:<\/p>\n<ul>\n<li><strong>In person<\/strong> \u2013 speak to our reception team at either practice<\/li>\n<li><strong>By email<\/strong> \u2013 <a href=\"mailto:b83012.manormp@nhs.net\">b83012.manormp@nhs.net<\/a><\/li>\n<\/ul>\n<p>Please tell us what information you would like to access. We may contact you if we need further information to understand your request or to confirm your identity.\u00a0There is <strong>normally no charge<\/strong> for a Subject Access Request.<\/p>\n<p>We will respond without undue delay and normally within <strong>one month<\/strong>. In some circumstances, such as where a request is complex, this period may be extended. We will let you know if this applies.<\/p>\n<details class=\"nhsuk-details nhsuk-expander\">\n\t\t\t<summary class=\"nhsuk-details__summary\">\n\t\t\t\t<span class=\"nhsuk-details__summary-text\">Accessing someone else's health record<\/span>\n\t\t\t<\/summary>\n\t\t\t<div class=\"nhsuk-details__text\"><\/p>\n<p>We have a duty to protect the confidentiality of every patient.<\/p>\n<p>If you would like access to another person&#8217;s health information, we will normally need their consent or appropriate legal authority before information can be provided.<\/p>\n<p>There may be different arrangements for children, people who are unable to provide consent, or someone acting with appropriate legal authority.<\/p>\n<p>Please contact our reception team if you would like to discuss accessing information on behalf of someone else.<\/p>\n<p><\/div>\n\t\t<\/details>\n<details class=\"nhsuk-details nhsuk-expander\">\n\t\t\t<summary class=\"nhsuk-details__summary\">\n\t\t\t\t<span class=\"nhsuk-details__summary-text\">Proxy access<\/span>\n\t\t\t<\/summary>\n\t\t\t<div class=\"nhsuk-details__text\"><\/p>\n<p>Proxy access allows another person to use GP online services on behalf of a patient.<\/p>\n<p>This can be helpful, for example, for a parent, carer or family member who helps someone manage their healthcare.<\/p>\n<p>Depending on the access agreed, a proxy may be able to help with things such as ordering repeat prescriptions, managing appointments or viewing parts of the patient&#8217;s health record.<\/p>\n<p>The level of access provided will depend on the individual circumstances and appropriate consent or authority.<\/p>\n<p>Please speak to our reception team if you would like to arrange or discuss proxy access.<\/p>\n<p><\/div>\n\t\t<\/details>\n<details class=\"nhsuk-details nhsuk-expander\">\n\t\t\t<summary class=\"nhsuk-details__summary\">\n\t\t\t\t<span class=\"nhsuk-details__summary-text\">Something in my health record is incorrect<\/span>\n\t\t\t<\/summary>\n\t\t\t<div class=\"nhsuk-details__text\"><\/p>\n<p>If you believe information recorded about you is inaccurate, please let us know.<\/p>\n<p>We can investigate information that may have been recorded incorrectly and make corrections where appropriate.<\/p>\n<p>Some information, particularly clinical information, cannot simply be deleted or changed because you disagree with it. Where appropriate, your concerns or additional information can be recorded alongside the original entry.<\/p>\n<p>Please contact the practice if you believe information in your record needs to be reviewed.<\/p>\n<p><\/div>\n\t\t<\/details>\n<details class=\"nhsuk-details nhsuk-expander\">\n\t\t\t<summary class=\"nhsuk-details__summary\">\n\t\t\t\t<span class=\"nhsuk-details__summary-text\">How we use and share your information<\/span>\n\t\t\t<\/summary>\n\t\t\t<div class=\"nhsuk-details__text\"><\/p>\n<p>We have a duty to keep your personal and health information confidential and secure.<\/p>\n<p>We use information about you to provide your healthcare and may share information where appropriate for your care or where there is another lawful reason to do so.<\/p>\n<p>For more detailed information about the information we collect, why we use it, who we may share it with and your rights, please read our privacy notice.<\/p>\n<p><\/div>\n\t\t<\/details>\n<details class=\"nhsuk-details nhsuk-expander\">\n\t\t\t<summary class=\"nhsuk-details__summary\">\n\t\t\t\t<span class=\"nhsuk-details__summary-text\">National Data Opt-Out<\/span>\n\t\t\t<\/summary>\n\t\t\t<div class=\"nhsuk-details__text\"><\/p>\n<p>The National Data Opt-Out allows you to choose whether your confidential patient information can be used for certain purposes beyond your individual care, such as research and planning.<\/p>\n<p>Choosing to opt out does not affect your individual care or treatment, and your information can still be used where there is a legal requirement or another permitted reason.<\/p>\n<p>You can check or change your choice at any time.
