Durham University Medical Evidence<\/h2>\n

If you require medical evidence for your college or academic team, if you have not already, you will need to speak to a GP to seek medical advice for the issue you are concerned about.<\/p>\n

There is a fee associated with this process as it is not an NHS service to provide medical evidence about you to the University. \n\t\t\t\t\n\t\t\t\t\n\t\t\t<\/svg>\n\t\t\t\tfees are listed elsewhere on this website<\/span>\n\t\t\t<\/a><\/p>\n

Once you have seen a GP and agreed that medical evidence can be provided, you need to complete a \u201cGreen Form\u201d . This grants your consent for the doctor to disclose information about your medical condition to the university.<\/p>\n

It is essential that a doctor has agreed in advance to providing evidence<\/b> and<\/b> you have been in touch with a doctor within the 2 months prior to the form being submitted. Any forms that do not meet this criteria will be rejected.<\/b><\/p>\n
