{"id":423,"date":"2025-10-07T20:38:27","date_gmt":"2025-10-07T19:38:27","guid":{"rendered":"https:\/\/nhs-demo.surgeryweb.org.uk\/?page_id=423"},"modified":"2026-02-09T09:14:49","modified_gmt":"2026-02-09T09:14:49","slug":"patient-participation-group","status":"publish","type":"page","link":"https:\/\/surgerywebtemp.org.uk\/a83011\/patient-participation-group\/","title":{"rendered":"Patient Participation Group"},"content":{"rendered":"<p data-block-key=\"8yl3d\">The practice has a \u201c<b>Patient Participation Group\u201d<\/b> membership of which is made up from patients registered at the practice who attend either the\u00a0Claypath Medical Centre\u00a0or University Health Centre.<\/p>\n<p data-block-key=\"c3fg8\">We are currently actively seeking new members to join the group to ensure a wider representation of our patient population.<\/p>\n<p data-block-key=\"1ulb3\">The aim of the group is that by working together we can continue to develop and improve the services we are able to offer patients.<\/p>\n<p data-block-key=\"7u839\">The <b>Patient Participation Group<\/b> meets several times a year and is made up of patients and practice staff.\u00a0\u00a0 If you are interested in joining this group please contact Gillian Bevan, General Manager, who would be happy to discuss this with you.<\/p>\n<h2 data-block-key=\"7u839\">PPG Terms of Reference<\/h2>\n<p data-block-key=\"5b5mo\">Overriding Aims and Principles<\/p>\n<ul>\n<li data-block-key=\"9l65j\">The PPG is not separate from the Claypath and University Medical Group but is a panel of patient volunteers who wish to work with the practice to provide practical support and to contribute to the continuous improvement of practice services, patient accessibility and the quality of care.<\/li>\n<li data-block-key=\"29vr9\">To promote patient perspectives and encourage the spirit of self-help amongst patients to improve their health and wellbeing e.g. practice initiative weeks.<\/li>\n<li data-block-key=\"amk1u\">To foster improved communication between the practice and its patients.<\/li>\n<li data-block-key=\"6sti9\">To actively promote the PPG with the aim of increasing Group membership and patient participation within the practice\u2019s population groups.<\/li>\n<\/ul>\n<p data-block-key=\"a173m\">1.\u00a0 \u00a0 \u00a0 \u00a0 \u00a0Meetings<\/p>\n<p data-block-key=\"4kbda\">1.1\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Meetings will take place every two months (excluding July and August). Practice General Manager and PPG representative to agree agenda for meetings involving practice representatives.\u00a0 \u00a0Agendas to be circulated by the practice 7 days prior to each meeting, together with relevant papers and minutes of the previous meeting. PPG representatives to agree agenda for meetings involving only PPG representatives.<\/p>\n<p data-block-key=\"ev3k2\">1.2\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Practice representation e.g. General Manager, Deputy Manager, Practice Doctor, to attend alternate meetings only.<\/p>\n<p data-block-key=\"dn6nm\">1.3\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Other staff may be asked to attend as appropriate. Any patients or staff, who wish to attend can do so at any time and are always welcome.<\/p>\n<p data-block-key=\"a2l55\">1.4\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 All meetings will usually be of approximately one and a half hours duration and held at a mutually convenient time to the majority of attendees. In the case of the meeting with practice representation, this is likely to be early evening for members with daytime work commitments and to ensure doctors\u2019 surgeries are not disrupted.<\/p>\n<p data-block-key=\"dfakr\">1.5\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Meetings with practice representatives to be held at the University Health Centre at 6.00 p.m. as staff available to lock-up the building.<\/p>\n<p data-block-key=\"1n236\">1.6\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 Meetings involving patient representatives to be held off practice premises or at the Claypath Medical Centre between 5.00 and 6.30 p.m. when practice cleaning staff available to lock-up the building.<\/p>\n<p data-block-key=\"s3exp\">2.\u00a0 \u00a0 \u00a0 \u00a0 \u00a0Objectives<\/p>\n<p data-block-key=\"42qpk\">2.1\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 To review with the practice representatives the format of the practice\u2019s annual patient questionnaires, to analyse the results, and incorporate solutions and improvements into an Action Plan to be monitored by the PPG.<\/p>\n<p data-block-key=\"cm5if\">2.2.\u00a0\u00a0\u00a0\u00a0\u00a0 To advise the practice team on the most appropriate ways to raise patient awareness and knowledge of existing services, new service development and other changes provided directly by the practice or in the community through the practice website or other media including flyers or notice boards.<\/p>\n<p data-block-key=\"es9\">2.3\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 To discuss ways the practice can directly promote public health issues e.g. obesity, diabetes, smoking, by positive signposting of services, health promoting sessions or self-sustaining groups etc.<\/p>\n<p data-block-key=\"95jsf\">2.4\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 To review and update a welcome pack for new patients and students.<\/p>\n<p data-block-key=\"f7kkg\">2.5\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 To review regularly patient access to services provided by the practice, those provided by the NHS as statutory services, those provided by the Local Authority and other voluntary organisations in the community.<\/p>\n<p data-block-key=\"4euro\">2.6\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 To be apprised of the current Practice Business &amp; Strategy Plan, Annual Report and Our Outstanding Care documents and identify ways the PPG can assist in Patient Engagement &amp; Experience.<\/p>\n<p data-block-key=\"3stm7\">2.7\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 To be made aware of and review local and national initiatives which may impact on the Claypath Medical Practice and its patients, to include a verbal report on the Central Durham Confederation and Durham Constituency<\/p>\n<p data-block-key=\"f98md\">2.8\u00a0\u00a0\u00a0\u00a0\u00a0 To work with North Durham Patient Reference Group by providing representatives, receiving information on developments at regional level and taking note of best practice in other GP practices which may be adopted by ourselves.<\/p>\n<p data-block-key=\"s3exp\">3.\u00a0 \u00a0 \u00a0 Code of Conduct<\/p>\n<ul>\n<li data-block-key=\"4844m\">The minutes of meetings to be displayed in practice waiting areas.<\/li>\n<li data-block-key=\"97so0\">Agenda items and group discussion will be on issues generally relevant to the organisation of practice services and facilities for patients. The group should not use meetings to raise individual patient health concerns or issues or to lobby on subjects not associated with the general running of the practice.<\/li>\n<li data-block-key=\"er8s4\">Members to respect patient confidentiality at all times.
